Health Risk Assessments

Adults age 50 and older can get a free health risk assessment through Stay Healthy Seniors.  The assessments are a terrific way for seniors to learn about their risk of developing certain medical conditions.

Those interested complete a series of questions about their current health and lifestyle.  Information from the assessment forms is then analyzed, and in approximately two weeks seniors receive a letter detailing the results of their assessment.  This includes what their risk is for developing diabetes, heart disease, high blood pressure and other health conditions.  Participants are then encouraged to follow up with their health care provider to discuss the results of the assessment.

Assessments can be completed at the Stay Healthy Center in Oakdale Mall, Johnson City, during regular business hours.  In addition, throughout the year Stay Healthy Seniors holds special assessment events at various locations in the community.
