Living Will/Advanced Directive

In New York State, family members can't speak for you if you become incapacitated as a result of an injury or illness, unless you have filled out an advance directive designating a family member to make health decisions for you.  The advance directive, known as the Health Care Proxy, is one of the most important ways patients can make their wishes known.  The state health department has a website where you can learn more about advance directives, and download the form that could give you and your loved ones peace of mind should a health crisis occur.
