Long Term Home Health Care
The Long Term Home Health Care Program of the Ideal Senior Living Center provides services to those who need ongoing care in the most comfortable environment possible
- their home.
As part of the admission process, our nursing coordinator and a case
manager will meet with the client, family or caregiver. This in-home consultation will help to determine which services will most benefit the client. As part of our coordinated network of care at the Ideal Senior Living Center, clients will continue to benefit through easy, convenient access to these services:
Our Long Term Home Health Care program offers each client the knowledge and experience of many healthcare professionals through our team approach. Under the direction of a registered nurse, our home care team creates an individual, detailed plan of care with the physician, client, and family. Long Term Home Health Care is about 75 percent of the average cost of care in a nursing home. Our services are fully covered by Medicaid, and partly covered by Medicare. Arrangements can be made for private pay as well. Ideal's Long Term Home Health Care program provides a personalized package of services for each client, while the client remains at home with family and near friends.
