How It Works
Use the Online Giving Form to make your gift or send your gift with the name of the tribute and information about whom to notify.

When we receive your gift, we will send a card to whomever you designate notifying them of your thoughtfulness.

Names of those honored by memorial or tribute gifts are recognized in the United Health Services Foundation Annual Report.  This is a wonderful way to remember or honor a friend or loved one.

Tribute gifts may be designated to a particular program or area of care or may be made to support the greatest need.
