URINE DRUG TESTING

(Pre-employment, random, for-cause, post accident, DOT, etc) All urine drug screens

processes to identify the use of adulterants.

BREATH ALCOHOL TESTING

All breath alcohol tests are performed by
certified Breath Alcohol Technicians and includes the cost of confirmation of
positive results. The evidential breath testing (EBT) equipment used for this service is
approved by the National Highway Safety Administration. Prior to testing, the individual
will be required to sign a testing form (U.S. Department of Transportation (DOT) or
non-regulated) Breath Alcohol Testing Form, which documents the individual will
submit to a breath alcohol testing, the name of the Breath Alcohol Technician conducting
the test, and the results of the test. The procedure for breath alcohol testing is
available on request.
Results will be given to the employer in a confidential manner. Such reports may be in
writing, in person, or by telephone or electronic means. The Breath Alcohol Technician
shall ensure immediate transmission of results that require the employer to prevent the
employee from performing a safety sensitive function.
MEDICAL REVIEW OFFICER SERVICES

Medical Review Officer (MRO) services are
available for interpretation of drug test results. The MRO is a licensed physician
knowledgeable in the medical use of prescription drugs and the pharmacology and toxicology
of illicit drugs. For all positive test results the MRO will conduct a medical interview
by phone or in person, including review of medical history or any relevant biomedical
factors, prior to reporting a positive result to the Company. The MRO will advise the
individual of the right to have the secondary split specimen tested if the request is made
within 72 hours of the notification of a verified positive result. If the MRO is unable to
reach the individual who has tested positive after several attempts over five (5) business
days, the MRO will notify the Company and request the Company ask the individual to
contact the MRO.
RANDOM PROGRAMS AND CONSORTIUM POOLS

United Occupational Medicine will provide
computerized randomization services for drug and/or alcohol testing at the
employers request. Testing is required at least quarterly to meet federal DOT
regulations. Employers not regulated under DOT regulations may set their own frequency and
numbers based on Company policy. It is the Companys responsibility to provide United
Occupational Medicine with an accurate employee list at the same frequency of the
requested random draws. United Occupational Medicine manages several Consortium Pools for
both large and small employers. The Company will receive quarterly and annual reports
indicating the number of employees tested and the number of positive tests.
WORKSITE NURSING SERVICES

United Occupational Medicine at United Health
Services Hospitals provides Worksite Nursing Services from four (4) to forty (40) hours
per week based on the Companys needs. Services are rendered by Registered
Professional Nurses experienced in Occupational Health Nursing. The Occupational Health
Nurse may provide the normal range of Company Nurse Services which may include:
Provide pre-employment and annual nurse assessments including height, weight, blood
pressure, vision testing, and urine multistix.
Coordinate and assist in scheduling the appropriate OSHA required medical surveillance
programs per Companys needs.
Audiometric programs, to the extent that Company equipment is available.
Sick call and first aid services, providing first aid services in accordance with
regulations and guidelines specified under the Occupational Safety and Health Act of 1970
and 29 CFR Part 1904.
Perform Ergonomic Evaluations of work stations and make appropriate recommendations.
Administration of approved medications in accordance with physician protocol.
Administration of immunization updates, facilitation of travel immunizations.
Participate, where appropriate, in Company Wellness Programs.
Active participation on Safety Committees, Ergonomic Committees, etc. The Occupational
Health Nurse may act as the Case Manager for Workers Compensation and Disability
Claims at the Companys request. Duties may include:
- Secure and review Injury
Reports and report with appropriate employee authorization pertinent information, as
defined by the Company, to the Safety Committee or as directed.
- Prepare and file
Workers Compensation Form C-2.
- Secure documentation, with
appropriate employee authorization, from medical providers on necessity for absence from
work due to Workers Compensation covered injury/illness.
- Monitor employees after
Workers Compensation covered injury/illness to reduce lost work time and assist
employee in early return to work.
- Assist the Company in
identifying light and/or modified duty assignments.
- Coordinate and communicate
with employees, physicians, department and insurance carriers the most feasible return to
work.
TUBERCULIN SKIN TESTING PPD (Mantoux)

United Occupational Medicine will administer
baseline, annual, and post-exposure Mantoux Skin Testing for the Company. The employee
will be questioned regarding previous skin testing results. If an employee reports a
history of a positive skin test, further tuberculin skin testing will not be performed.
Those employees reporting positive skin test will complete a Tuberculosis Screen. If there
are no physical symptoms, nothing further will be required. If there are positive findings
from the questionnaire, employee will be referred to their primary care physician or the
Broome County Health Department Chest Clinic.
The intradermal skin testing is done with 0.1 ml of 5 TU PPD test material (Tubersol),
using a tuberculin syringe with a 27 gauge needle. The test must be interpreted by United
Occupational Medicine 48 to 72 hours after administration. If the individual does not
return within the 72 hour period, the test will be repeated on the other arm after seven
(7) days or more after the initial skin test. Interpretations will be made according to
the Positive Tuberculin Skin Test Table. Individuals who are positive
converters will be referred to their private medical doctor or the Broome County Health
Department Chest Clinic. Testing following a suspected exposure to tuberculosis will occur
as follows: if a Mantoux Skin Test has not been done within the past 12 months, a baseline
test will be done and then repeated twelve (12) weeks post-exposure.
HEPATITIS B IMMUNIZATIONS

United Occupational Medicine will provide a
series of Hepatitis B Immunizations to all employees designated by the Company and
desiring the vaccinations. The vaccine is a series of three (3) injections given over a
six (6) month period. Prior to receiving the vaccine, each recipient will be given the
opportunity to have any questions or concerns about the vaccine answered. Each recipient
will be required to sign a United Health Services Hospitals Hepatitis B Immunization
Consent/Declination form prior to receiving the vaccine.
