GENERAL HEALTH ASSESSMENT EXAMINATION (New Hire, Annual, Periodic)

RESPIRATOR CLEARANCE EXAMINATIONS (29 CFR 1910.134)

Respirator Clearance Exams are performed prior to the employee being fit tested or required to use a respirator and to current employees who annually give positive responses to any questions among question 1 through 8 of the below sited questionnaire.

The following services will be routinely performed as part of a Respirator Clearance Examination:

  • OSHA Respirator Medical History Questionnaire
  • Review of Medical/Occupational History
  • Medical Exam and Counseling Report
  • Height /Weight
  • Blood Pressure
  • Pulmonary Function Screen
  • Urine Dip for sugar and protein.

RESPIRATOR CLEARANCE EXAMINATIONS FOR OCCUPATIONAL EXPOSURE TO TUBERCULOSIS (29 CFR 1910.139)

All individuals will be required to complete an OSHA Respirator Medical Evaluation Questionnaire and Tuberculosis Screen. Based on the review of the questionnaire and screen, if no pertinent medical conditions are identified, the employer will receive the “Medical Screening Statement”. After review of the above questionnaires, individuals with a positive history and/or symptoms will be scheduled for a Respirator Clearance Examination.

The following services will be routinely performed as part of a Respirator Clearance Examination:

  • Review of Medical/Occupational History
  • Medical Exam and Counseling Report
  • Height /Weight
  • Blood Pressure
  • Pulmonary Function Screen
  • Urine Dip for sugar and protein.

ASBESTOS MEDICAL SURVEILLANCE (29 CFR 1910.1001)

The following services will be routinely performed for Asbestos Medical Surveillance Examinations:

  • Administration of the Asbestos Initial Medical Questionnaire Part 1 or Part 2 (Periodic Exams)
  • Asbestos Medical Surveillance Employee Questionnaire Report
  • Medical Exam and Counseling Report
  • Height/Weight
  • Blood Pressure
  • Stool test for occult blood
  • Pulmonary Function Screen In addition, a two-view chest x-ray will be performed according CFR 1910.1001 recommended frequency table and pre-assignment to occupation with asbestos exposure.

All chest x-rays will be interpreted by a radiologist with “B” Reader Certification and according to the International Classification of Radiographs for Pneumoconiosis (LO-U/C).

FORMALDEHYDE MEDICAL SURVEILLANCE (29 CFR 1910.1048)

The following services will be routinely performed for Formaldehyde

  • Medical Surveillance Exams:
    • Completion of initial “Formaldehyde Medical Surveillance” Questionnaire or an Update
    • Medical Exam and Counseling Report
    • Height/Weight
    • Blood Pressure
    • Urine Dip for sugar and protein
    • Pulmonary Function Screen HAZARDOUS MATERIALS (HAZMAT) RESPONSE TEAM EXAMS (29 CFR 1910-120) The following services will be routinely performed as part of a HAZMAT Examination:
    • OSHA Respirator Medical Evaluation Questionnaire
    • Review of Medical/Occupational history
    • Height/Weight
    • Audiogram, Pulmonary Function Screen
    • Chemistry Profile
    • Electrocardiogram for age 35 and over
    • Physical Examination
    • Blood Pressure
    • Vision
    • Complete Blood Count
    • Routine Urinalysis

    Lead Medical Surveillance(29 CFR 1910.1025)

    The following services will be routinely performed as part of a Lead Medical Surveillance Examination:

    • OSHA Respirator Medical Evaluation Questionnaire
    • Review of Medical/Occupational History
    • Height/Weight, Pulmonary Function Screen
    • Chemistry Profile, Blood Lead, Physical Examination
    • Blood Pressure
    • Complete Blood Count
    • Routine Urinalysis
    • Zinc Protoporphyrin

    Hazardous Waste Operations Examinations (29 CFR 1910.120)

    The following services will be routinely performed as part of the Hazardous Waste Operations Examination:

    • OSHA Respirator Medical Evaluation Questionnaire
    • Review of Medical/Occupational History
    • Height/Weight
    • Audiogram, Pulmonary Function Screen
    • Chemistry Profile
    • Electrocardiogram
    • Serum PCB, Physical Examination
    • Blood Pressure
    • Vision
    • Complete Blood Count
    • Urinalysis, Blood Lead
    • Stool for Occult Blood
    • Trglycerides
    • Liver Profile
    • Urine Heavy Metal Screen
    • RBC Cholinesterase.

    AUDIOMETRICS (29 CFR 1910.95)

    United Occupational Medicine will perform baseline and annual audiograms for the Company. All audiograms will be performed by a certified hearing conservationist. Any abnormal audiograms which demonstrate a standard threshold shift will be reviewed by a certified, hearing conservationist with recommendations being forwarded to the Company.

    The employee will be asked to complete and sign a “Hearing Conservation Program” form.

    In accordance with OSHA Standard 29 CFR 1910.95 (g) (8), if a comparison of the annual audiogram to the baseline audiogram indicates a standard threshold shift, the employee shall be informed by the employer of this fact in writing within twenty-one (21) days of the determination. The Company will be responsible for complying with this OSHA Standard.

    Please note: Individuals scheduled for testing must not be exposed to any loud noise environment (e.g. bowling, motorcycles, target shooting, rock concerts, etc.) for a minimum period of fourteen (14) hours prior to testing; however, hearing protection may be used as a substitute to exposure to work place noise. BLOOD BORNE PATHOGEN POST-EXPOSURE FOLLOW-UP (29 CFR 1910.1030) United Occupational Medicine has the ability to provide follow-up care for anyone who has a work- related exposure to blood or body fluids. The following services are available to meet the requirements OSHA now mandates:

    PRE-EXPOSURE

    • Hepatitis B Vaccination: a series of three (3) injections administered at 0, 1 and 6 months.
    • If requested, pre-vaccination Hepatitis Profile can be done to determine antibody status.
    • Pre-exposure Hepatitis B Vaccination and/or antibody status will be billed to employers at the prevailing rates.

    POST-EXPOSURE

    • Twenty-four (24) hour access for initial post-exposure services.
    • Post-exposure medical evaluation and follow-up counseling.
    • Antiretroviral prophylaxis for high risk exposures.
    • HBV status and update.
    • Confidential HIV counseling and testing. The staff at United Health Services Hospitals are trained HIV Counselors. In addition, United Health Services Hospitals Infection Control Nurses are available as needed for consultation.
    • The OSHA required employer report of medical evaluation will be forwarded to the designated company representative.

    All records are maintained in a confidential file.

    BLOOD BORNE STANDARD TRAINING AND EDUCATION (29 CFR 1910.1030)

    The Company must develop an Exposure Control Plan that identifies all employees with occupational exposure, the procedure for evaluation of exposure incidents, a schedule for implementing all provisions of the standard and educate employees on company plan/policy/procedure. This can be done before or in coordination with/after United Occupational Medicine training. United Occupational Medicine staff can provide the following training and education:

    • Explanation of the OSHA Blood borne Standard.
    • Explanation of Hepatitis B and its implication(s).
    • Explanation of Hepatitis B Vaccine.
    • Explanation of HIV and its implication(s).
    • Presentation of a video which defines work place exposure and a question and answer period.
